[HG DOLFIN] Bug fix in ODECollection, use to_double when printing reals

Add utility function compute_vertex_map for computing a mapping between
the vertices of two meshes based on the mapping "global vertex indices"
for each of the meshes. SubMesh demo seems to work fine now.
