Regarding the file format XYZ. 
It is just one file with plain/raw "x y z" values.
It works for 2D meshes ("x y") plus a "z=f(x,y)" function. 

If I remember, I just simplified another file format, 
messing with the minimum number of files and code possible.
Regarding the empty foo.xyz file it is useless, on the other hand, is
there any problem if a list of the foo######.xyz is kept in foo.xyz, as
in the pvd/vtu format?
