So far as  I can tell, this only happens with click apps. So if I do
"Di" I get one entry for dialer, and 2 for FateDice.

  Multiple entries for each application

Status in “unity-lens-applications” package in Ubuntu:
  Confirmed

Bug description:
  In each part of the application scope, applications are listed
  multiple times. When doing a search, etc... See screenshot for an
  example in the Installed lens. This also happens when doing a search.
