I've had this happen twice.  Each time was slightly different.

I have dual monitors and the first time it happened, I moved my mouse to
the other screen.  When the user login box switched to the screen my
cursor was on, the password field appeared.  When I moved my mouse back
to the second monitor, this time, the password field was there.

 The second time it happened, both screens were the same with no
password field.  Moving the cursor back and forth never reloaded the
password field and I had to take other measures to log in.

Bug description:
  Sometimes (but not always) after locking the screen there is the
  situation not having any input field where I can type the password to
  unlock the session again. The shaded gray area simply does not have
  the input field. I can't do anything, I have to switch to text console
  and stop/start lightdm service to cure the problem loosing all of my
  session :(
