[Bug 2117221] [NEW] "et al." inconsistency

Thanks for the work you did on this, I have a lot of scentific pdfs
which are now easier to manage. However, whereas Alt A on A B et al.
finds A B, it drops the original A B et al. Likewise, Alt A on A B & C D
et al. hits A B, C D, A B & C D, and (the original) A B & C D et al., it
does not find A B et al. or C D et al. I'm a great fan of Calibre, and
appreciate all you do, so sorry to be nitpicking, but I'm sure you can
see why the missing hits would be better included.
