Slave servers during upgrade should be started with --skip-slave-start in addition to skip grant

Before upgrade, make a clean shutdown, if you have orphan tables, drop

I'm able to reproduce your crash with percona server 5.5.57-38.9 to
5.6.37-82.2 upgrade.

or misconfigured. This error can also be caused by malfunctioning hardware.

something is definitely wrong and this may fail.
