Updates for latest CVS version of FIAT. FFC now uses shapes_new
instead of shapes. Only one minor change: dims --> dim.

All forms in src/demo/ verified to give correct results.

Rob, if you like you can go ahead and change the reference elements
and I can make sure FFC works with the new reference elements.
