[HG FFC] Add new demo form FacetIntegrals for testing facet integrals in tensor

description:
Add new demo form FacetIntegrals for testing facet integrals in tensor
representation. Old PoissonDG demo will not work with new UFL-based
tensor code generator since it contains a division (1/h). Division,
square root etc will not be handled by tensor representation.
