  USB devices occasionally stop working

Bug description:
  I can't report this with ubuntu bug because:

  $ ubuntu-bug
  Segmentation fault (core dumped)

  so here's my best description:

  Sometimes, within a few minutes of boot, my usb-connected mouse and
  keyboard still stop working. For the mouse, it just stops but for the
  keyboard it breaks after a key down event which leaves whatever key
  you pressed repeating for ever (which makes it quite hard to reboot!).

  Quite often I will know this is about to happen because the media keys
  and scroll wheel on the keyboard fail to work for a while before the
  eventual failure.

  I see this in the syslog:
