1.Is this really a failure?
Yes
2.Is it reproducible? If not, how often would it be reproducible?
No really, about 1/3 resume will fail, especially when the image is large.
*IMPORTANT*: the resume more likely to fail when the image is large.
3.Did it work before on a prior kernel in the same release, or prior release?
Yes, Ubuntu 14.04.3 with 3.16.0-40-generic #54~14.04.1-Ubuntu SMP Wed Jun 10 17:30:45 UTC 2015 x86_64 x86_64 x86_64 GNU/Linux works well.
4.Do the symptoms vary at all between hibernation attempts? For example, one time WiFi didn't work, but another a kernel error shows on the screen.
No, every time is the same.
5.Do you end up with a flashing Caps Lock light?
No, totally hang
6.Please advise on how the computer hibernated, and resumed specifically. For example:
Executing at a terminal pm-hibernate, s2disk, echo disk > /sys/power/state
Clicking the word Hibernate in the GUI.
The result are same
7.Did the machine break while going into hibernation or waking up?
Hibernate is fine, waking up fail.
8.Did you see any error messages leading up to the failure?
No, just below output with 3.19

Incomplete
Bug description:
With kernel 3.19.0 and 4.1.13, there is a 1/3 probability of resume
failure.
If the resume failed, system will hang with the console output
below(with no_console_suspend)

intel_pstate CPU 2 exiting
smpboot: CPU 2 is now offline
intel_pstate CPU 3 exiting

I have just downgraded to 3.16.0-40, it works well with about 5
hibernate/resume cycle, but I will do more test on it.
