Bug description:
  Thinkpad T420 with nvidia nvs 4200M video card. Discrete card is
  activated and Optimus is disabled in BIOS.

  Send the machine to sleep.

  Wake Up. Only a blank screen and non-responsive system. Only solution
  is to hard-reset the computer.

  After new boot up, received notification of a failed suspend/resume
