The log seems to indicate disk write errors, implying either a kernel
bug or a disk failure.

ubuntu 14.04.4 installation on Firestone baremetal with "Guided - use entire disk and set up LVM" fails when ext3 is chosen as / file system. Below error message is displayed.

Steps to Reproduce

1. Start netboot installation on Firestone baremetal machine from
http://ports.ubuntu.com/ubuntu-ports/dists/trusty-proposed/main/installer-ppc64el/current/images/wily-netboot/
2. During "Partition disks" select
Manual -> Guided partitioning -> Guided - use entire disk and set up LVM

I see the following errors in the syslog, parted is able to create the partition but mke2fs is unable to create the ext3 file system.
