  ath3k Bluetooth driver will drop after hibernate for about 15 times

Bug description:
  CID: 201401-14508 Dell Inspiron 3048 AIO

  The Atheros Bluetooth [0fc3:e005] on this system will stop working
  after hibernated for about 15 times, it 100% reproducible.

  Step:
  1. Install 14.04.2 + update (3.16.0-39)
  2. hibernate it with fwts, command:
  sudo fwts -P s3 --s4-multiple=15
  3. Check if the Bluetooth still working

  Expected result:
  * Bluetooth works fine as expected

  Actual result:
  * Bluetooth stopped working after hibernate stress test, with error message in dmesg output
