Now I can not resolve this problem.
I use another Wi-Fi+BT card.
But may be my story will be usefull for future work with problem.
I have Acer TM B113-E which complected with Broadcom Corporation BCM43228 Wi-Fi+BT card.
With older kernels(3.11) it works fine. But when I began upgrade kernel, this card works unstable(with Cisco wi-fi routers I have a lot of broken connections, some times connection was droped on another routers too). I purchase Atheros AR9485 and successfully use it until have upgrade to 14.04.
With new OS I can not set up BT to work fine. I probe all methods, but nothing helps me.
If I remember, I put hands down and insert original BCM43228. I did not setup it too and reinsert AR9485. After this series of reinserting Atheros card began works fine!
I think Broadcom card may be wrote some information to BIOS ACPI (or other part of BIOS) and new card began work without troubles.
I probe this method now. But I can not resolve my problem by this manual.
Now I use Broadcom card, which works some stable, but has slow speed(near 1Mb/s). Atheros card had 2.5Mb/s speed on this router. After advanced setuping of my home router, I had increase of speed to 1.5-1.9Mb/s on it.

I have Qualcomm Atheros AR9485 Wireless Network Adapter [168c:0032]
(rev 01) on Acer TM B113-E notebook.
When I use ath9k with 3.11 kernel then all works fine. WiFi and Bluetooth works as well.
I have problem with bluetooth when install some later kernels(I'm test 3.15rc1 and (3.13 or 3.14)) from http://kernel.ubuntu.com/~kernel-ppa/mainline/
After this installation I have not bluetooth device! When I switch Wi-Fi by acpi key, I can switch only Wi-Fi to on or off state(I saw status by "rfkill list"). Bluetooth device will not reconize for me...
But if I will power off notebook and disconnect battery, then after power on I'm get working Bluetooth.
I could not pay attention to this troubles but now we have Ubuntu 14.04(I tested on Kubuntu 14.04).
After installed Kubuntu 14.04 I have working only Wi-Fi. Power off+disconnect battery and other probes(such as I probe to install non-free firmware) do not have results. System do not recognize BT device... I can not see this device in rfkill list and in KDE's System Settings->Bluetooth...
In 3.11 kernel and later kernel I have problem with bluetooth after sleeping too. Module do not start after resume. I wrote script with manual starting of btusb module and get working BT after resume.
