Can you see if this issue still happens with the latest upstream 3.2
kernel[0]?  If it does, we can perform a kernel bisect to identify the
commit that introduced this regression.

  [Lenovo ThinkStation C30 1097] Boot failure with kernel image 3.2.0-63
  in 12.04

Bug description:
  After upgrading to 3.2.0-63 from 3.2.0-61 I can no longer boot, due to
  a failure to mount root. Selecting the old 3.2.0-61 kernel in grub
  fixes the issue.

  I get an "Unable to reset I T nexus?" error in the kernel log, which seems to be related to the commit 584ec1226519 "isci:
  fix reset timeout handling". See the debian discussion here: https://lkml.org/lkml/2014/4/28/164
