Kernel oops: USB Failed to set U1 timeout

Bug description:
I get a Kernel oops that affects the usb driver. After the oops, all
usb ports are dead and reboot seems not to work properly, too.
I cannot provoke the oops to happen, but it occurs about once or twice
a week, apparently after the computer goes to power saving mode and
turns off the screen. There is a usb hub built-in the monitor.
Dmesg output is attached, ubuntu-bug linux outputs are below.
