Both Pcbnew and CvPcb exhibit the same behavior when launched by KiCad
and when I run the executable directly by double clicking the executable
and opening the net and board files using the recently opened menu.

Yes.  It is an existing project that I opened in KiCad and launched
CvPcb to assign the footprints.  I verified the project fp-lib-table
file was correct.
