Since my last comment this branch has added the following:

- Restores the default SIGPIPE behaviour in subprocesses.

- Runs rabbitmqctl once before starting the server to ensure that
  there is no race to create the cookie file.

This seems to be reliable, but I am going to spend some more time
verifying that before putting this back up for review.
