Allow me to answer with another question...
When does this chain ever stop? :-)
I think that assuming things are healthy is good because:
1. It doesn't highlight "everything is ok", as that should be the
common state.
2. It likely makes problems more visually distinctive.
As for the implementation, I don't think it makes a difference,
though I would err on the side of having to deal with fewer icons. I'm
more bringing this up as I think it's likely a better UI, rather than
implementation concerns.
The answer is likely to implement both and then decide (or perhaps just
doctor screenshots once we have the basic card display in place, and
decide based on those, as that's almost as good.)
