this is on purpose because the command that corresponds to +cfg needs to be removed and re-added, as its caption will have been changed because that caption shows previews of the current setting values. it could be individually removed and re-added, but that would make it be added to the last position, loosing the alphabetical sort in the user command menu.
therefore, all commands are removed and re-sent.

perhaps in the future, when we have an improved user command standard
that allows better customization of menu item placements and dialog
creations, this can be looked into again...

It seems that when using latest access.lua all UCMD commands are resend to the clients on a topic change .... :)
