The file was missing from both kind of finished items' tabs or just from one of them? 
The finished items is built up from events sent by the logmanager after every segment downloaded so it would be nice to see the Downloads.log in case of you can reproduce this problem.

  Completed downloads are not always displayed in Finished Downloads Tab

Bug description:
  Just like the title says. Sometimes I don't see the entry for finished items in the Finished Downloads tab, however the files can be found in their download location. I don't know whether there's a connection or not, but I recently saw this happing with files that lost some of their sources (users got dropped because of slow download speed), although I manually readded them later.
  Additional note: the downloads in such cases were performed with segmented downloading.
