This seems to be happening in auth/xmlrpc.

I have added functionality to the mentioned patch that will make the
file name unique, e.g Profile Picture, Profile Picture.1, Profile
Picture.2, ... ;)

Please let me know if you have any other suggestions for this.

  Unable to rename profile icon file

Status in Mahara ePortfolio:

Bug description:
  I found myself having all five profile icon files with the same
  default name "Profile Icon". This makes it difficult to pick the
  required icon quickly when adding it to a Page/View. Can these files
  be renameable? Or am I just missing something obvious?
