  Symptoms:
  
  Clicking on edit in Page view using FF and Chrome either closes the file
  browser window, or in Content > Files view shows a Loading... display
  and nothing happens.  This invokes the PHP error above.
  
  Mahara Demo site does not exhibit this problem (is it running 1.4.1?)
  
  Does not seem to be connected to Bug #819102 or Bug #891436 although
  similarities exist.

+ wondering if something is missing/corrupt from pieforms.  I forgot to
+ mention the problem appeared after upgrading Mahara.
