Changes made that fixed the problem above.

 - Overwrote folder mahara/lib/pieforms with installation files.  
 - Checked on home computer WinXP & Chrome 16.0.912.75 - OK.

Tested on work computer and issue remained:

 - Cleared browser cache
 - Cleared OpenDNS cache

All working again :-)

Recommendation:  If you received PHP errors regarding Pieforms, replace
pieforms folder on server with corresponding folder from installation
package.

  Symptoms:

  Clicking on edit in Page view using FF and Chrome either closes the
  file browser window, or in Content > Files view shows a Loading...
  display and nothing happens.  This invokes the PHP error above.

  Mahara Demo site does not exhibit this problem (is it running 1.4.1?)

  Does not seem to be connected to Bug #819102 or Bug #891436 although
  similarities exist.

  Found this: http://mahara.org/interaction/forum/topic.php?id=2306, so
  wondering if something is missing/corrupt from pieforms.  I forgot to
  mention the problem appeared after upgrading Mahara.
