  Add institution staff rights during registration approval

Bug description:
  When a new user registers for an institution that requires approval,
  the institutional admin should be able to assign staff permissions
  during the approval process, based on the user's (confirmed) email
  address.  This saves an inconvenient additional step of waiting until
  the new user has logged in.
