This was a mistake due to the myportfolio block_instance table been
slightly different to  normal one

i had to manually drop index that was not needed now

 drop index "blocinst_viecolord_uix";

there is a local /db/update.php patch to handle this problem

  Logged in as admin 
  theme = moderation 
  ----------------------------------
  "Selected a page (Page 2). Selected the last layout under 'Basic options' on the Edit layout tab.
  Error message shows when trying to add a second image to the bottom left hand side of the page."
