Add "Skip to main content" link (Bug #1259741)

Added a hidden link to the beginning of all pages which becomes visible on
focus and allows keyboard users to bypass the navigation and move focus to
the main content of the page.

Title:
  "Jump to Content" link should be included for screen reader users

Status in Mahara ePortfolio:
  In Progress

Bug description:
  To meet current accessibility standards there needs to be a "Jump to
  Content" link to allow screen reader users to bypass navigation. The
  link should set focus to the start of the main content (the first <h1>
  or the content div)

  Note that since our navigation is well-structured this is not
  necessary for WCAG Level A, so this should be a wishlist item.
