Add hidden label to site search box (Bug #1262867)

Add new option to pieforms element, 'hiddenlabel', which adds the class
'accessible-hidden' to the element's label. Used this setting in the
universal search form for both internal and elasticsearch plugins.

  Site search box does not have a label

Bug description:
  For people using screen readers, having a label for every form input is essential as they have no other (easy) way of knowing what the input is for. Currently the site search box does not have an associated label.
  A hidden label should be added which contains the search box placeholder text (either "Search users" or "Search" depending on which plugin is used)
