Use labels in email address element (Bug #1277297)

Linked email labels to radio buttons (and added element title as hidden
text). Also added the 'btn' class to the add email button

  Radio buttons in email selector need labels

Bug description:
  For people using screen readers, having a label for every form input is essential as they have no other (easy) way of knowing what the input is for. In the email list pieforms element (used on the Profile page) emails are displayed as radio buttons with labels, but the labels are not linked to the actual buttons.
  The labels should probably use the same method as normal radio button elements and include both the element title and the individual email.
