  Buttons should have nicer focus style

Status in Mahara ePortfolio:

Bug description:
  Would be cool to use the 'hover' button style when buttons have focus
  rather than the default dotted outline - for some buttons (eg. green
  ones in the default theme) the outline can be quite hard to see. The
  same goes for icon buttons, although a new style would have to be
  added for them.
