Bug 1512926: Theme style guide

Examples are currently spurious, this is just to flesh
out the technical underpinnings. Requires some assets
via a CDN to keep the dependancies minimal.

URL is /theme/styleguide.php and user must be logged in.

A link to to the style guide shoulde be added to themeing
docuementation once the guide has been populated with the
componenents we want to include.

Bug description:
  Mahara should have a theme style guide to make creating themes and
  plugins easier and also allow for more consistency across Mahara.

  The style guide will be available directly in Mahara and accessible
  via a URL.
