Bug 1545359: Added control to disable protocols switches
In web service configuration, when service provider master switch
and service requester master switch are set to off,
then all protocol switches are set to off and disabled.

Individual web services switches need to be disabled if master switch
is off

Bug description:
When the web services master switch is set to "No", the individual
protocols can still be changed to "Yes", which is misleading as that
suggests that they would still work.
They should be disabled. Jono created a disabled state for the
switches for bug #1489700 that should be used.
