  Change "Doesn't meet the standard" icon to a more distinct one

Bug description:
  Mahara 16.10RC1

  The "Doesn't meet the standard" icon (white circle with red border) is
  too close to the "Ready for assessment" icon (white circle with blue
  border). We need something a bit more distinguishable for
  accessibility and decided to go with the times-circle icon (red circle
  with white cross).
