  Remove code for old notifications system

Status in Mahara:
  In Progress

Bug description:
  Back in Bug 1497065 we make the multirecipient notifications the
  default plugin. So that we now do not use some forms/pages for the old
  notifications system.

  We should clean up this code to make things less confusing
