Bug 1795597: Install/upgrade LTI tables for mysql

Moved the adding of indexes to post install so we can differentiate
between postgres and mysql as they need to set up the indexes on
'text' fields differently

  Issues relating to LTI when upgrading from 18.04 to 18.10RC1

Bug description:
  I don't know if there is a bug or it is a problem on my server.

  Mahara 18.04.2 is running fine on my server without error messages in
  apache error.log.

  Ubuntu 14.04 apache2 & mysql server on the same machine.

  Patch-Level: 01.10.2018

  Upgraded the application by console command as described in your wiki.
  sudo -u www-data /usr/bin/php /var/www/html/admin/cli/upgrade.php

  I got the following error in terminal (attached as file)

  Any idea ?
