In admin menu click on Configure site-> Site options
(/admin/site/options.php)

When loading the page I see the error message:

[WAR] 94 (lib/form/elements/autocomplete.php:124) Undefined index:
institution


this error comes from the changes made in the patch 1fa6bfc59cdaa1d7895619ee3bae062cf5edfd11
