Title:
  When setting up an institution, put the focus on the first form field
  'Institution name'

Status in Mahara:

Bug description:
  When you add an institution to the site, the first form field is not
  focussed, requiring you to click into it before you can get started.
  It would be better to focus it directly. It is also an accessibility
  improvement.

  To replicate:

  1. Go to Admin menu -> Institutions -> Settings -> Add.
  2. On the next page you see the form.
  Expected result: The field 'Institution name' is focussed (and thus has the field outline).
  Actual result: Nothing is focussed.
