Bug description:
My story
==========
As a portfolio viewer I want to remove myself from a portfolio so that
I do not see the portfolios on my list of portfolios to which I have
access.
How
==========
**Important**: This applies only to access that has been given to a
person themselves, not public, registered accounts, group access, e.g.
view access table has user column set with an ID.
* When a portfolio has been shared with a person, that person sees an additional option on each portfolio page via the 'More options' button: A trash icon in red and the text 'Remove my access' (not in red).
* When a portfolio has been shared with a person, that person can remove their own access on 'Shared with me' by clicking the 'Remove my access' icon.
* When the icon / button is clicked, a confirmation modal is displayed that states that access is lost to the entire portfolio.
* The modal contains a message field allowing the viewer to provide a reason (similarly to the 'Objectionable content' modal).
* Notification is sent to the portfolio author with the decision by the reviewer.
* Viewer is removed from the access list for the portfolio.
* The date and time when the viewer removes themselves is recorded in the event log.
