Bug 1938741: Unsubscribe/subscribe to forum missing a 'group' value

Add the group value expected by the unsubscribe function into the form

Bug description:
  Clicking Unsubscribe from within a forum throws and error. It is
  missing a 'group' value in the pieform.

  To test:
  1) Create a group
  2) Go to 'forums' tab
  3) Click 'New forum' button
  4) Set a title and description then save
  5) Click the 'Unsubscribe from forum' button

  Expected result - get unsubscribed
  Actual result - get warning "Undefined index: group"
