Title:
  Change the keyboard focus to have four sides around the focussed
  element

Bug description:
  In certain places in Mahara, the keyboard focus is only shown on three
  sides instead of four when an element is focused, e.g. on the
  dashboard in 'My portfolios'. This fix changes that in the places that
  we could determine already.
