i haven't yet, but if you've seen it, please file a bug!
bonus points for a reproducable test case ;)
to find out we can probably register a SIGUSR1 handler that does  
nothing but counting the signals received and then signal the process  
in a tight loop while keeping it busy. the numbers should match at the  
end of the test.
