Glad it was an easy solution.  In the future, all mythtv and mythplugins
and mythweb bugs should be filed against the 'mythtv' source package.
We don't use a 'mythweb' source package anymore.

** Package changed: mythweb (Ubuntu) => mythtv (Ubuntu)

** No longer affects: mythtv

  mythweb fails with PHP error

Status in “mythtv” package in Ubuntu:
  Invalid

Bug description:
  After a new installation of myth (backend, frontend and mythweb all on
  this machine - default installation, apart from allowing remote
  frontends), mythweb fails with the following:

  Warning at /usr/share/mythtv/mythweb/modules/_shared/tmpl/default/header.php, line 12:
  !!NoTrans: Cannot modify header information - headers already sent by (output started at /usr/share/mythtv/mythweb/modules/_shared/tmpl/default/header.php:13)!!

  The apache error log contains:
