Setting up mythtv-database (2:0.26.0+fixes.20121118.340b5d4-0ubuntu1) ...
ERROR 1046 (3D000) at line 22: No database selected

Not sure how I missed this bug report. Basically, you have invalid
credentials in your config.xml and/or mysql.txt file in /etc/mythtv/
