  Create an opportunity from a customer form view

Bug description:
  In OpenERP 6.1, when I want to create an opportunity directly from a
  customer view (in Action menu), I don't find email and phone records
  if they are defined. But it works well if a create an opportunity in
  Sales > Opportunity.
