I have attached another screenshot showing the error when I tried the
same procedure to install base_report_designer.zip on OpenOffice 3.4.1
WINDOWS so I guess it's not a Mac specific issue.

  base_report_designer extention does not work using MacOS & OpenOffice
  3.4.1

Bug description:
  I am trying out base_report_designer with Apache OpenOffice 3.4.1 for
  Mac OS X.

  I've downloaded the base_report_designer.zip from apps.openerp.com
  (12/4/12)

  I follow the procedure to install the extention in OpenOffice.

  After installing the extention, OpenOffice reports that the status of
  the Extention is unknown.

  However I do see the OpenERP report designer in the menu of
  OpenOffice.

  I try to enter the server parameters but it does not work. After
  clicking on the command, there is no prompt for me to enter a working
  openerp server.
