  Creation of new song in Song Editor is sometimes not possible

Bug description:
  Hello!

  Sometimes it is not possible to create a new song and a new
  Topic/Author/Book. Let me describe how do reproduce this:

  1) Create a new song, enter a verse, the title and add an author.
  2) click "Manage Authors, Topics, Song Books"
  3) Add a new Author/Topic/Book (<- this will fail)
  4) close the "Song Maintenance" window
  5) Do not save the song (click "Cancel")

  Two things have gone wrong a) step three failed and b) you are now
  unable to add a new song.
