Appears the "index" used to display options does not take into account
"inactive" items.  This is really apparent if the only active plugins
are "Presentation" and "Custom"

  New

Bug description:
  I have the following plugings enabled:  Song, Bible, Custom.

  When I open tools/configure openlp and select "Custom", the option for
  "Media Display" is shown.

  If i activate "Presentation", the correct options are displayed.
