The image manager only shows the file name, not the path. It won't be possible to differentiate the two if they have the same name, other than by the small thumbnail.
Currently if you do pick the same filename twice from different locations you end up with the name duplicated and clicking on both instances in the manager displays the same image.

Tim's proposal at least prevents this until a correct solution is possible, and it doesn't cause any regression as far as I can see...
