I just checked it out and here are some suggestions:
The caption should be something line "Default Service Name" (analog to "Default Image")
Layout: Use the QFormLayout (the input widgets are staying in QHBoxLayouts). Lables are "Date and Time:", "Name:" and "Example:" (the context can be known from the caption)
Description: It does not have to be on the GUI. These informations should be present in the manual and maybe in a tooltip. The first two sentences are nice. As the user already sees a example it would be better to list the wildcard and describe their role. The last sentence is unnecessary as there is no advantage in using no default.
