Note there is related configuration option: General > Background Audio > Start background audio paused
But there isn't any configuration option for media items.

  Inconsistency media and backing track controls

Status in OpenLP - Worship Presentation Software:

Bug description:
  Sending a media item live, the item goes live stopped. You need to
  click the play button to start it.

  Sending a song with backing tracks live, the backing tracks are
  paused, and you have to "un pause" to play.

  We should make this consistent with each other. This would become more
  pronounced if the Play and Pause buttons are merged on the media item.
  Either we should send all live items is the stopped condition, or they
  all should be started paused.
